Telehealth Informed Consent & Billing Consent

Effective date: October 25, 2026

What you are consenting to

MyTeleHealthPoint provides care to eligible patients physically located in Ohio, primarily by secure asynchronous messaging. Ohio-authorized clinicians are available 24 hours a day, seven days a week. Our stated average is 30 minutes to a meaningful initial clinician response after a complete eligible request; actual waits vary. This is neither an emergency response system nor a promise that a visit, prescription, or treatment will be completed within 30 minutes.

A clinician may ask questions, request photographs or records through the secure clinical system, use video or telephone when appropriate, order testing, or recommend in-person evaluation. A clinician may decline to treat remotely if that is necessary to meet the standard of care.

Benefits, limitations, and alternatives

Remote care can reduce travel and give you access outside usual office hours. However, messaging, photographs, video, and patient-reported information may not provide the information a hands-on examination would. Technical failures, delayed messages, incomplete details, and testing delays can affect assessment. Alternatives include in-person primary care, urgent care, specialty care, or emergency services. No medication or requested outcome is guaranteed. Our standard prescription-refill service does not provide controlled-substance refills.

Emergencies

Do not wait for a telehealth reply for chest pain, serious breathing difficulty, stroke symptoms, uncontrolled bleeding, severe allergic reaction, immediate suicide danger, or another emergency. Call 911 or seek emergency care. For mental-health crisis support, call or text 988. The clinician may recommend urgent in-person care after reviewing your request.

Identity, clinician information, and minors

You agree to provide accurate identity, age, current physical location, contact details, relevant history, medications, and allergies. The treating clinician will identify their name and professional license type as required. A parent or legal representative generally must provide consent for a minor unless a service-specific exception applies. We verify the legal authority and confidentiality rules before care. Some services require a different clinical setting or clinician type.

Privacy, results, and follow-up

Use the designated secure channel for medical information. Protect access to your device and account. We document the clinical encounter and, when indicated, testing, referrals, and results. Routine review and communication of tests ordered during your encounter do not automatically create another consultation charge. A new or materially changed concern may require another evaluation, and any applicable fee will be explained before proceeding. Follow the plan you receive and seek reassessment if symptoms worsen.

Consent to receive telehealth treatment

I understand the benefits, limitations, risks, and alternatives to telehealth. I have had an opportunity to ask questions. I voluntarily agree to a remote clinical evaluation using the method the clinician considers appropriate. I know I may decline or withdraw consent for future services, subject to care already furnished and lawful recordkeeping.

Separate consent to be billed

I have been shown the consultation price, applicable payment and refund terms, and the possibility of additional medication, laboratory, collection, or outside-service charges. I separately consent to the disclosed professional consultation fee. I understand that prescriptions are not guaranteed and that any increase or separate nonurgent fee must be explained and accepted before the additional service.

We do not impose a telehealth facility, origination, or provider-site equipment fee.

How consent is documented

Before clinical services and any charge requiring consent, the practice records the consenting person and authority, relevant policy versions, date/time, disclosed fee, acceptance, and required identity/location information in its secure clinical record. Merely visiting this webpage or clicking a navigation link does not create informed clinical or billing consent. If the secure workflow cannot capture these records, contact support before continuing.
